Support coordination works the same way under the NDIS everywhere in Australia. What changes from state to state is everything around it: the health system, the schools, housing, guardianship, and the practical reality of distance. A coordinator who knows Queensland well can save you months. This guide explains how support coordination works for Queensland participants, and how we approach it from our base in Kallangur.

What a support coordinator does

A support coordinator helps you understand your NDIS plan, find and connect with the right providers, keep those supports working together, and build your own confidence in managing it all over time. For the full picture, read what a support coordinator actually does.

In a typical week, that can mean:

The three levels of coordination

Your plan will fund one of three levels. Support Connection is short term help to connect with community, informal and funded supports. Coordination of Supports is the most common level, providing ongoing help to use your plan and coordinate providers. Specialist Support Coordination is for complex or high risk situations, often involving several systems at once, and is usually delivered by a coordinator with a relevant tertiary qualification.

The Queensland context

A good Queensland coordinator knows how to work across the state systems that sit alongside the NDIS. These are the ones we deal with most often.

Health

Hospital discharges, mental health services and specialist clinics run through Queensland Health and its hospital and health services. Coordinating a safe discharge home, with NDIS supports in place on day one, takes early contact with the ward team and the social worker.

Education

For children and young people, school is often where supports have to work hardest. Coordinators help families link therapy and behaviour support with what the school provides, and plan carefully for the move from primary to high school and from school to adult life.

Housing

Housing pressure in South East Queensland is real. Coordinators help with Supported Independent Living, Specialist Disability Accommodation and, where needed, links with social housing. Supporting evidence from a coordinator can be important in urgent housing applications.

Guardianship and decision making

Where someone needs support to make decisions, Queensland guardianship and administration matters go through QCAT, and the Office of the Public Guardian may be involved. Coordinators work with guardians and families to keep the participant at the centre of decisions.

Child Safety and justice

For participants involved with Child Safety or the justice system, specialist coordination is often needed to keep every agency aligned and to make sure NDIS supports continue through changes in placement or circumstances.

Distance

Queensland is big. Providers are much easier to find in Brisbane than in regional towns, and waitlists vary a lot by area. Part of the job is knowing where capacity actually exists and when a mix of in-person and telehealth services makes sense.

How we work with Queensland participants

Our team is based in Kallangur, so we work in person across Moreton Bay, Brisbane, Ipswich, Logan, the Gold Coast, the Sunshine Coast and surrounding areas, and by video and phone further afield. What that looks like in practice:

Where behaviour support is part of the picture, our coordination and behaviour support work closely together, which makes a real difference for families managing complex needs.

What to expect in your first month

The first few weeks set the tone for the whole plan. Expect your coordinator to go through the plan with you line by line, confirm your goals, and agree how you want to communicate. They will then start connecting providers, set up service agreements, and make sure any urgent gaps are covered first. By the end of the first month you should know who is doing what, how your budget is tracking, and what happens next.

Choosing a Queensland coordinator

Ask about their local provider knowledge, their experience with the systems above, how they build evidence, and how quickly they respond. Our guide on how to choose an NDIS support coordinator has the full list of questions and red flags.
